Presenting Clinical Problem
A middle-aged woman presented with extensive dental deterioration requiring comprehensive restorative intervention across multiple quadrants.
- Dentinal Hypersensitivity: Generalised tooth sensitivity indicating exposed dentine or compromised enamel
- Enamel Fractures: Multiple visible craze lines throughout anterior dentition
- Severe Dental Attrition: Advanced tooth wear extending into dentine on anterior teeth
- Posterior Restoration Failure: Large amalgam restorations with secondary caries and structural cracks in molar teeth
- Acute Infection: Dental abscess requiring emergency endodontic intervention
The primary clinical objective was to address acute infection, restore structural integrity throughout the dentition, and implement protective measures against further deterioration.
Treatment Plan & Clinical Process
A comprehensive staged treatment approach was implemented over an extended period, beginning with emergency care and progressing through systematic restoration.
Phase 1: Emergency Intervention: Root canal therapy was performed to address acute dental abscess. Following endodontic treatment, the affected tooth received crown restoration to protect remaining tooth structure.
Phase 2: Treatment Planning: Digital Smile Design technology facilitated comprehensive treatment visualisation. Clinical assessment evaluated extent of wear patterns, crack propagation, and restoration failure. At-home teeth whitening was implemented to establish optimal baseline tooth colour.
Phase 3: Anterior Restoration: Eight porcelain veneers were placed on anterior teeth to restore appropriate crown length, protect worn dentine, and address visible crack lines. Minimal preparation protocols were utilised where tooth structure permitted.
Phase 4: Posterior Restoration: Seven porcelain crowns were placed on posterior teeth, replacing failed amalgam restorations and protecting cracked tooth structure. Crown preparation removed compromised tissue and secondary caries whilst maintaining adequate tooth structure for retention.
Phase 5: Protective Measures: Custom occlusal splint was fabricated to protect restorations and remaining natural tooth structure from parafunctional forces and prevent recurrent wear patterns.
